7th Grade Immunization Requirements

All children entering 7th grade are required to:

1. Have one (1) dose of the Tdap (tetanus, diphtheria, and pertussis), and (1) dose of the meningococcal conjugate vaccine. 

2. Receive vaccinations and obtain the Georgia Certificate of Immunization (Form 3231) from a health care provider. 

3. Or an Affidavit of Religious Objection must be one file

Vaccinate your child early to beat the back-to-school rush!     

An updated 7th Grade Immunization form will be required before the student is allowed to attend classroom instruction. Please let us know if you have any questions.
